Title:  CARE MANAGER

Reports to:  CARE MANAGER SUPERVISOR 


OVERVIEW

RESPONSIBILITIES:


     Care Management 

Work with the medical provider team with all patients of the practice, this would include the following:


Maintains strict confidentiality and follow HIPAA regulation

Complete Bliss care manager assessment identifying needs and create an action care plan when needed. 

Ensure patient receives medication the same day of visit if available, based on the test and treat model. 

Complete Patient Assistance Programs for scheduled patients if needed.

Working with medical providers to create action care plans when needed and report them the outcomes. 

Follow up with patients and their action care plan to ensure goals and needs are met. 

Pre-visit planning to verify if patients are insured/covered for services needed.

Educating the patient about self-management tasks they can undertake to gain greater control of their health status. 

Focuses on treatment adherence and medical compliance based on the test and treat model.

Manage many aspects of the patient’s care: referrals to specialists, housing, food, and other community resources based on identified needs in the care management assessment.

Create a list of all patients seen on a weekly basis.

Participates in professional development activities.

Performs other related duties as required.
